As a courtesy, we will coach you with important questons to ask your insurance carrier about your insurance benefit. You will be given a completed insurance claim form with all the information that you need to file your own insurance claim. Just put it in an envelope, stick a stamp on it, and send it in the mail.
A sliding fee scale is available for those experiencing financial difficulties. Discounts are also available when you pre-pay for the month's sessions.
